The FCVS and Uniform Application forms may look similar but they are separate forms. The FCVS affidavit is sent to FCVS. The Uniform Application affidavit is sent to each board you are applying to. If you send the Uniform Application affidavit to FSMB, there will be a delay in your licensure process.
